High quality, effective and efficient primary care is composed of several elements such as accessibility and coordination, implementation of health care professional practices and patient engagement in self-care. These elements were already identified in 2003 as major challenges of primary care research. However, they are still recognized as essential and this speaks to the need  to address those issues using innovative research strategies such as interprofessional research, patient-oriented research and effective knowledge translation.

This research program aims to achieve three objectives: 1) To optimize the trajectory of care for people with chronic diseases seen in primary care; 2) To support patients with complex care needs and high users of services and interprofessional teams in shared decision-making in primary care and; 3) To develop training interventions for health professionals working in an interprofessional context. The research program is part of an approach to optimize primary care, conduct patient-oriented research and engage in knowledge translation with patients and knowledge users.
